A Certificate of Insurance that includes:

Any listed exclusions
Listing USA Truck Inc as the certificate holder
General Liability of at least $1,000,000.
Auto Liability of at least $1,000,000.
Cargo Coverage of at least $100,000.
Trailer Interchange of at least $25,000.
DOT safety score of “satisfactory,” or “none”
